MI Community Care is a free program run by Washtenaw and Livingston County nonprofits that work together to meet the health and personal goals of participants–many of whom have complex lives and health conditions. Each MI Community Care participant is paired with a care coordinator from one of several local organizations. Care coordinators meet with participants to learn about their health and personal goals, then help participants achieve those goals. Over the last few years, hundreds of residents have benefitted from this free and confidential program, which does not interfere with other forms of aid.

MI Community Care is a free community-wide care coordination program run by regional health, behavioral health, and social service organizations. The goal: To improve the health and well-being of people with complex needs by providing long term, coordinated, and multidisciplinary care management. Hundreds of Livingston and Washtenaw County residents have benefitted from the program, which began in 2016 with Affordable Care Act funding.
